It sounds like you need to sit down with a dentist and let him know what you concerns are and what you want to accomplish dentally. This way the dentist can do an exam to determine the best way to go from where you are to where you want to go. Since it sounds like you feel you have a lot going on you will need at least a comprehensive exam with current xrays. For any significant cosmetic changes additional records such as photographs and models will be needed in order to plan so that the final results will look good in the face upon completion.
