Case Description
I have two persistent problems can you help what I should anticipate with these two and if there is treatment for these? 1) I have been grinding inside of my upper front teeth for few years. I am now feeling an impression in back of these teeth. Is there a cure to fill these impressions? Is there something that will stop this? 2) While eating, my tongue gets in the way many times and I bite it. It hurts and at times it bleeds. What should I expect for treatment? Thanks. HS

HS:There can be at least three explanations for the wear on the backs of the front teeth: 1. Interfering fit of the back teeth causing the jaws to avoid in chewing and work in a more exaggerated chewing pattern by coming forward in chewing striking the upper teeth.2. A misfit in jaw alignment where the lower teeth are too forward (or upper teeth too far back, or even a combination) where the tooth position is in the way of a normal chewing pattern and the system is trying to adapt (thru wear, loosening of teeth, or attempting to chew in a more retruded position which is very difficult to do).3. Just plain grinding of the teeth subconsciously or in sleep.So in answer to your question can this be fixed? Yes, of course. But the better question is will it hold up? That depends on finding the cause and managing it. Otherwise, you will just destroy the repairs. You should see a dentist for a thorough evaluation to determine the cause. At the same time the dentist should evaluate your bite to present you with a solution to the problem you are having with tongue biting. The dentist should explain to you his/her findings and present treatment options so that you may make an informed decision for treatment. Good luck!Jim Hinkle, DDS MAGD

Hello,Have you asked your last dentist about this 2 problems? It sound to me like you will need a custom nightguard. Do you grind at night or in the daytime? To answer your other question about while you eat, you are biting on your tongue, there is a guard to prevent tongue thrush so you do not bite your tongue. You will need an exam to determine these problems further and to confirm these answers.
